Medication Refills
If you are an established patient of ours, you can now request medication refills without having to go through the phone and phone menu system. The requests will have to be faxed to us either via your computer or, you can print out the form and then fax it through a regular fax machine.
To get an HTML (browser) page which you can fill out by typing in your information, click the link below. You can save this page to your computer for future use if you select "Save As" in Internet Explorer. If you have a modem in your computer with FAX capabilities, you can fax this directly to us. If your computer does not have a FAXmodem and you wish to send it through a regular fax machine, you can fill out the following page and print it and then fax it to us. Printing several blank pages for future use is also an option!
Please make note of the following:
- if you aren't an established patient in our office, don't even try it!
- if you haven't been seen for over 1 year, you may need to be re-evaluated before we will issue a refill
- you must fill out all of the information which is marked with an asterisk on the request form. Failure to do so may delay and/or prevent timely processing of your request.
- requests which are received after 4 PM on Friday will not be processed until the next business day (Monday, unless there is a holiday)
