We know you have many choices when choosing a dermatologist in Clive, IA, so we have made requesting an appointment a simple and convenient process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Insurance and Billing
We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own dermatologist.
We accept the following Insurances:

We do not participate with Unity Point Insurance or Humana Medicare- Out of network benefits would apply.
We accept checks, cash or credit cards (Mastercard, Visa, Discover, Care Credit). We also offer a flexible payment plan and Care Credit. Please see our Financial Coordinator for details.
Co-payments are due at the time of service. If our office does not participate with your insurance, full payment is due at the time of service. More info on co-pays
Referrals are necessary if required by your insurance plan and we must have the referral number before you are seen. Many insurances will not allow retroactive referrals after you have been seen by us. If we are required to call and obtain your referral number at the time of service, this can result in a significant delay in your appointment time, and might require rescheduling of your appointment in some cases..
